Application for Services
If you wish to receive a copy of our Application for Vision Services, request via email at SevierLions@gmail.com or pick up a paper copy from Eye Designs Optical, 1240 Fox Meadows Boulevard, Sevierville, TN, 37862. Directions for how to return the form are explained on the application.
All completed applications will be reviewed by our Vision Committee and applicants will be notified if services are approved.
